The Soteria programs give our seniors the tools to continue to maintain their independence and live at home safer and more connected. It serves patients’ and their families’ ease & wellbeing by providing the physician care team with a combination of data received from RPM devices and lab results and the advantage of face-to-face communications with patients using live video over their own television. This gives the care team a fuller picture of the patients’ health allowing them to act timely, and provide preventative care, early intervention or even emergency services as needed. "Soteria-360: A Lifesaving Solution for Managing High Blood Pressure – G.L.'s Success Story"
G.L. was managing high blood pressure, but even with Lasix diuretics, his condition was unstable. Before a scheduled virtual visit, the nurse practitioner reviewed G.L.'s vital signs using the Soteria Remote Patient Monitoring (RPM) device. Noticing erratic and dangerously high readings, the nurse practitioner collaborated with G.L.'s primary care provider. Together, they adjusted his medication regimen, which stabilized his blood pressure, effectively preventing an emergency room visit and potential hospitalization.

While his wife was out running errands, L.S. experienced a fall at home. Fortunately, he was wearing the Soteria issued Personal Emergency Response (PERS) device so he immediately pressed the emergency button for help. The monitoring team responded and sent an EMT who arrived promptly, helped him up and assessed his condition. By the time the patient’s wife returned, he was stable and comfortable.

"A Life-Saving Virtual Visit: How Soteria-360 Helped Detect a Critical Condition for H.S."
H.S. lives alone, but his son visits once a week to be present during virtual visits. While preparing for their virtual care visit, the nurse practitioner (NP) reviewed the patient’s vital signs in the Soteria dashboard and was concerned with the inconsistent readings. During the video visit, in addition to troubling vital sign data, the NP became increasingly concerned with the changes she could see from past video visits. The NP urged the son to take H.S. to the hospital promptly to have more extensive testing. The son wrote to the physician several days later, thanking the doctor and his team for “…saving his dad’s life” since they found bleeding in the patient’s brain, requiring emergency surgery.

"Urgent Care When It Mattered Most: How Soteria-360 Helped J.P. Avoid a Critical Health Crisis"
J.P. was discharged from the hospital and enrolled in the Soteria program, in addition to regular dialysis treatments. The nurse practitioner (NP) reviewed his vital signs and was concerned enough that she contacted a physician who did an emergency virtual care appointment at the end of the day on a Friday. J.P. told the doctor that was he feeling very poorly and that, while he had tried to reach the dialysis center, they were unresponsive. The physician then arranged for him to be treated on Saturday morning at the dialysis center where they removed seven liters of liquid.
